How Much Does It Cost to Open a Med Spa? A Line-by-Line Budget
A med spa budget is not one number. It is fifteen lines, each with its own range. Here is the line-by-line budget from published franchise disclosures and industry data, plus the brand and launch line most founders leave out.

Most founders ask what it costs to open a med spa and get one number back. One number is not much use. A med spa budget is fifteen or so separate lines, each with its own range, and the total depends on which lines you inflate and which you starve.
This guide breaks the budget down line by line, using figures from published franchise disclosure documents, industry data, and vendor pricing. It is written for a founder building a first location, whether you are a nurse practitioner, a physician, or an operator partnering with a medical director. Every number below is linked to where it came from.
Why franchise disclosures are the best public benchmark
Independent med spas do not publish their opening budgets. Franchisors are required to. Item 7 of a Franchise Disclosure Document lists the estimated initial investment for a new location, line by line, with a low and high estimate.
Two med spa franchise systems give a useful spread:
- dermani MEDSPA estimates a total initial investment of 435,654 to 860,625 dollars in its 2025 FDD, as summarized by Zee Report.
- VIO Med Spa estimates 642,722 to 1,109,954 dollars, as summarized by SharpSheets.
Those totals include franchise fees and training costs you would not pay as an independent. Strip those out and the remaining lines are a fair picture of what it takes to open a branded, fully built med spa in a leased retail or medical space. A founder starting in a smaller suite with one injector and no energy devices can open for far less. A founder opening a large flagship with several lasers can spend more.
For context on the revenue side, the American Med Spa Association's 2024 State of the Industry recap reported average annual revenue of 1,398,833 dollars per location in 2023, an average of 245 patient visits per month, and an average spend of 527 dollars per visit. Those are averages across established locations, not a first-year forecast.
The line-by-line budget
Ranges below are drawn from the two franchise disclosures unless otherwise noted. Franchise fees and franchisor training are excluded.

| Build-out (leasehold improvements) | $83,397 | $400,000 | dermani low [1], VIO high [2] |
| Furniture and fixtures | $22,019 | $33,476 | dermani 2025 FDD [1] |
| Signage | $5,503 | $15,041 | dermani 2025 FDD [1] |
| Devices and treatment equipment | $117,362 | $335,000 | dermani low [1], VIO high, which includes furniture [2] |
| Initial product inventory | $36,437 | $49,839 | VIO low [2], dermani high [1] |
| Computers, POS, and software setup | $7,650 | $29,540 | dermani low [1], VIO high [2] |
| Licenses and permits | $700 | $5,000 | VIO low [2], dermani high [1] |
| Insurance (initial premiums and deposits) | $3,500 | $14,500 | VIO low [2], dermani high [1] |
| Professional fees (legal, accounting) | $4,000 | $25,000 | dermani low [1], VIO high [2] |
| Grand opening marketing | $20,000 | $50,000 | dermani low [1], VIO high [2] |
| Working capital (first three months) | $40,000 | $120,000 | dermani low [1], VIO high [2] |
| One-time total, approximate | about $364,000 | about $1.13 million | Sum of lines above |
Ongoing monthly lines that start before opening day:
| Monthly line | Low | High | Source |
|---|---|---|---|
| Medical director | $1,000 | $5,000 | Moxie's observed range [3] |
| Practice management software | $159 | $364 | Boulevard plan pricing on Capterra [4] |
| Malpractice / professional liability (median) | about $208 | varies | Insureon median for spa businesses [5] |
The high total slightly double counts, because VIO's equipment line includes furniture and fixtures. Treat the table as a frame for your own quotes, not a quote.

The lines, explained
Space and build-out
Build-out is the largest and least predictable line. Matthews, a commercial real estate brokerage, puts the national average cost to develop medical office space at 150 to 300 dollars per square foot for interior build-out and conversion. A second-generation medical suite with plumbing in the right places costs far less to convert than a raw retail shell. Negotiate the tenant improvement allowance before you sign, and do not assume it covers the whole job.
Devices
Devices are where budgets break. AmSpa's guide to buying med spa devices notes that medical aesthetic technology can cost as much as 250,000 dollars, and warns that the price tag does not tell the whole story: consumables, maintenance, warranties, recertification fees, and resale restrictions all belong in the decision. Moxie lists a laser or IPL system at 50,000 to 75,000 dollars new. Leasing lowers the upfront line but adds a monthly payment that has to be covered by bookings from the first month.
Inventory
Neurotoxins, fillers, skincare for retail, and treatment consumables. The franchise estimates land between roughly 36,000 and 50,000 dollars. Opening inventory should match the menu you can actually staff, not the menu you hope to grow into.
Insurance
You need general liability, property, professional liability for every provider, and workers' compensation once you have employees. Insureon reports median premiums for spa businesses of about 2,500 dollars a year for professional liability and 624 dollars a year for general liability. Injectors and laser providers often pay more depending on services and limits, so get quotes against your actual menu.
Medical director
Most states require physician involvement in some form, and arrangements vary widely. Moxie reports that most medical director agreements it sees fall between 1,000 and 2,500 dollars a month, occasionally reaching 5,000. How that agreement is structured, and whether it is legal in your state, is a question for a healthcare attorney before you sign, not after.
Software
Booking, charting, payments, and memberships usually live in one practice management system. Published plans range from about 12 dollars per user per month for Aesthetic Record to 159 to 364 dollars per month for Boulevard's tiers. Add the cost of a phone system, email marketing, and reviews software.
Working capital
The franchisors both budget three months of additional funds. Mindbody's guidance on opening a med spa suggests saving enough to cover three to six months of expenses. The low end assumes bookings ramp quickly. Most new locations do not ramp on schedule.
The brand and launch line
Franchisees inherit a brand. An independent founder has to build one, and this is the line most budgets leave out entirely. The ranges below are wide because the work is: a single-provider practice in a mid-sized market and a multi-room flagship in a major metro need different scopes.
| Brand and launch item | What it covers | Published benchmark |
|---|---|---|
| Positioning and strategy | Who you serve, what you lead with, how you differ from the three med spas nearest you | Usually scoped inside a branding project |
| Naming and trademark | Name development, clearance search, federal filing | USPTO base application fee of 350 dollars per class, plus attorney fees |
| Brand identity | Logo, type, color, photography direction, signage and print system | Clutch reports most branding projects at 10,000 to 49,999 dollars, with a reviewed average of 71,652 |
| Website | Treatment pages, booking integration, local search foundations | Clutch reports a 38,105 dollar average and a typical range of 2,000 to 100,000 |
| Launch content | Original photography and video of the space, providers, and treatments | Varies by market and day rates; get quotes |
| Pre-launch campaign | Waitlist, founding memberships, opening event, paid social | Franchise grand opening budgets of 20,000 to 50,000 dollars [1] [2] |
Treat those as reference points, not prices. Rates vary by market, by scope, and by who is doing the work. What matters more is the order: positioning and a name come first, because the signage, the website, the domain, the social handles, and the opening campaign all depend on them. Our med spa business plan guide covers how to write the brand and marketing sections so lenders and partners take them seriously.
Where founders overspend
Devices bought for the menu you hope to have. A second or third energy device purchased before you have the patient volume to fill it ties up cash for years. Each device carries a monthly payment, consumables, and a trained provider. Open with the treatments your team can book from week one, and add devices once demand is proven.
Build-out finishes no patient will notice. Spend on the treatment rooms, lighting, and the first ten feet inside the door. Custom millwork in a hallway does not book appointments.
Too much inventory. Product expires. Order to your staffed menu.
Where founders underspend
Brand and launch. A founder who spends 200,000 dollars on a laser and 3,000 dollars on a logo has built a clinic nobody has heard of. The brand is how patients find you, choose you over the established practice down the road, and come back. It also shapes how your website and local search presence perform from day one.
Working capital. Rent, payroll, the medical director, and the device lease all start before revenue does. Running out of cash in month four, with a full schedule finally in sight, is how good med spas close.
Professional fees. Entity structure, the medical director agreement, and state compliance are cheaper to get right before opening than to fix after a complaint.
A realistic way to build your number
- Get two or three build-out quotes on the actual space before you sign the lease.
- Price your opening menu, not your five-year menu, and size devices and inventory to it.
- Get insurance quotes for each provider and each service.
- Put the brand and launch line in the budget as its own line, with real quotes.
- Add at least three months of working capital, more if your market is crowded.
- Hand the result to an accountant who works with medical practices.
What belongs to someone else
Ownership structure, medical director agreements, scope of practice, and state licensing belong with a healthcare attorney in your state. Lending, projections, and tax structure belong with an accountant who works with med spas. Device selection belongs with your clinical team and your medical director. A brand agency's job is the line that makes the rest of the investment visible to patients.
Next step
If you are budgeting a new med spa, put the brand line in early, while the lease and the legal work are underway, so the name, identity, and website are ready when the doors open. That is where our launch work for new med spas begins, and our med spa branding page shows what the identity work includes.

How much does it cost to open a med spa?+
Published franchise disclosures put the initial investment for a fully built med spa at roughly 435,000 to 1.1 million dollars including franchise fees. Excluding those fees, the line items add up to roughly 364,000 to 1.13 million dollars. A smaller suite with injectables only and no energy devices can open for much less. Your number depends on space, devices, and market.
What is the biggest cost in opening a med spa?+
Build-out and devices are usually the two largest lines. In the dermani MEDSPA and VIO Med Spa disclosures, leasehold improvements range from about 83,000 to 400,000 dollars and equipment from about 117,000 to 335,000 dollars. Both depend heavily on the condition of your space and how many energy devices you buy before opening.
How much working capital does a new med spa need?+
Both franchise systems reviewed here budget three months of additional funds, between 40,000 and 120,000 dollars. Mindbody suggests saving enough to cover three to six months of expenses. Rent, payroll, the medical director, and device payments start before revenue ramps, so the higher end is safer in a crowded market.
How much does a med spa medical director cost?+
Moxie reports that most medical director agreements it sees fall between 1,000 and 2,500 dollars a month, occasionally reaching 5,000. Rules on physician supervision and ownership differ by state, so have a healthcare attorney in your state review the agreement and structure before you sign.
How much should a med spa budget for branding and a website?+
It varies by market and scope. Clutch reports most branding projects between 10,000 and 49,999 dollars and an average web design project of about 38,105 dollars, while franchise grand opening budgets run 20,000 to 50,000 dollars. Treat these as reference points and get quotes against a defined scope.
